OFFICE FEES AND SERVICES

Initial Office Visit - $275.00 (1-1/2 hours to 2 hours).
I use a Functional Medicine approach to assess your current health status and support the development of your future optimal health and well being. This approach identifies alternations in a body system or organ that might be susceptible to, though not yet be in, a disease state. After a thorough analysis, nutritional and lifestyle changes can rebalance your system so that you do not progress to a disease state. During your first visit, a comprehensive case history report is completed and the metabolic assessment and daily diet history forms are discussed. Additionally, your current and past lifestyle factors are discussed. I will then suggest tests or lab work that will assist me to complete the best assessment of your current health status. After a thorough evaluation, vitamins, minerals, herbs, and other natural products are used to re-balance your body systems.

Follow-up Office Visits - $70.00 to $110.00 (20 to 40 minutes).
During these visits we discuss your test results, a treatment plan, or any further testing that might be required so you can continue to improve your health.

Phone Consultations - $70.00 to $110.00 (20 to 40 minutes).
I am available to answer questions about your test results or treatment plan. These phone calls are usually no more than five to ten minutes. If you require additional time to discuss your test results, treatment plan, or any further testing that might be required it is a follow-up visit.

Services Available
The panel on the right side lists some of the frequently used tests. These tests determine the functional status of your endocrine, immune, digestive, or energy production systems. Your case history, metabolic assessment forms, and health status will determine which test is appropriate for you. The cost of a test depends on the number of individual panels ordered. You pay the Lab directly for each test that is ordered.

Insurance Information and Reimbursement
I belong to a Professional Co-op that allows my patients access to major lab services for blood and urine testing for a fraction of the regular cost. The Co-op allows me to consolidate the most valuable individual blood tests into panels of tests. These custom panels are non-reimbursable tests for insurance purposes. It may be possible to get reimbursement for some of the individual components of the panels depending on the type of policy you have.

Your insurance and reimbursement are between you and your insurance carrier. I do not file insurance forms as I am not on any insurance carriers’ preferred doctor list. Some office fees and/or tests may be covered by your insurance carrier. Please check your policy or call your insurance company and ask about the services that are covered by your policy. If any services are covered, I will provide you with a receipt (Superbill) that you can send to your insurance company for reimbursement.

Office Cancellation Policy
If you are unable to keep your appointment, kindly give 24 hours notice to avoid being charged for a cancelled or broken appointment.

As a courtesy to patients who have chemical sensitivities, please do not wear any perfume, cologne or other scented products when you visit our office. Thank you.

 


FREQUENTLY USED TESTS:

Saliva Hormone Function Test – A comprehensive hormone profile that measures active hormone levels. The test provides information about adrenal function, estrogen, progesterone, and testosterone levels plus additional immune factors.

Blood Chemistry Test Panel – A comprehensive blood panel that includes profiles to measure heart health, rate of aging, and inflammation in the body.

Functional Blood Nutrition Analysis Test – A comprehensive report is generated based on your Blood Chemistry Test Panel. The results comprise an analysis of your individual blood chemistry patterns and your specific biochemistry. This is the latest scientific approach for identifying nutritionally significant information based on your blood test values.

Comprehensive Thyroid Blood Panel which includes TSH (thyroid stimulating hormone) and Vitamin D Test – Thyroid gland function and auto-immune thyroid disease are evaluated.

GI Function Test – Problems with digestion, nutrient usage, colon waste elimination, and pathogens are determined. Poor digestion and malabsorption can lead to immune dysfunction, nutritional insufficiencies, food allergies and other toxicities, and various disease states.

Organic Acids Comprehensive Profile – Measures key components in your cell’s biochemical machinery and provides information about how efficiently your metabolism is functioning. Also evaluates how well your cells are converting the food you eat into energy and products for a healthy brain and body.

Food Antibody Profile 90 – Testing for delayed reactions to 90 different foods. Delayed food reactions can contribute to or exacerbate several health problems including fatigue, weakness, food cravings, muscle and joint pain, skin problems, gastrointestinal symptoms, heart and lung symptoms and psychological problems.
